                  Introduction                                          

Transitions is a community-based private not-for-profit organization providing rehabilitation, therapy, counseling, consultation, education, advocacy and support services to persons with mental disorders, their families and the general community.  The goal being to provide those we serve with the tools necessary to live productive, safe, independent and healthy lives.

                   Philosophy                                            

  • Transitions believes that recovery is a real possibility for every individual.

  • Transitions is dedicated to the belief that regardless of severity of the illness, all persons have the right to receive quality services.

  • Transitions recognizes that services should be provided according to individual need and desire.

  • Transitions believes that choices of services, treatment, venues and activities should be provided whenever possible.

  • Transitions recognizes its obligation to maintain confidentiality and protect the individual rights of every person we serve.

Transitions is dedicated to promoting, enhancing, and improving the health, recovery, and well-being of individuals, families, and the community impacted by mental health issues.

Above all, we value respect for and dignity of every individual.  We are dedicated to quality, continual growth and positive outcomes for clients, staff, customers and community.

                      Funding                                              

For 26 years, Transitions has provided critical services and opportunities to persons with mental disorders including housing, employment, employment training, advocacy, counseling, consultation, therapy and support.  For 26 years we have brought hope to otherwise empty and isolated lives.  But in order to keep doing the work to which we have dedicated ourselves; there must be sufficient funding to meet the many needs of these very deserving individuals.
