POSITION/TITLE: Employment Services Coordinator

AGENCY PROGRAM:Rehabilitation Services

DESCRIPTION:

  • Provide support and consultation to staff and clients
  • Assess client needs, progress and issues relating to vocational services
  • Contact area businesses regarding potential IPS placements
  • Provide vocational training/coaching and evaluation of progress to clients according to Individual Recovery Plan objectives
  • Monitor and review client service activities of the program.
  • Maintain accurate records in all phases of evaluation, training and placement, completing all documentation in an accurate and timely manner
  • Ensure that IPS is meeting fidelity per Best Practice standards
  • Participate in the hiring, orientation, training, supervision, performance evaluation and employment termination of Employment Consultants
  • Represent Transitions on local committees as assigned
  • Participate in DRS staffings
  • Participate in development and implementation of IEP for each client
  • Responsible for ensuring that criteria for monthly, quarterly, semi-annual and annual reports for Employment Program: IPS, SEP and 708 is complete, accurate and submitted to the Rehab Director on time.
  • Uphold all client rights as identified in agency polices
  • Activity participate in agency meetings/training and workshops or conferences as approved and arranged
  • Keep Director of Rehabilitation informed at all times
  • Read and responsibly follow/use all guidelines/information in Transitions Policy Manual, Procedure Manual and Program Manuals

QUALIFICATIONS:

  • Bachelors degree in human service field from an accredited college
  • 2-4 years documented experience in a human service field – or comparable qualifications as determined by the CEO
  • Demonstrated computational, statistical and analytical skills
  • Demonstrated skills in crisis management
  • Knowledge and experience working with persons with prolonged mental illness desirable
  • Supervisory experience preferred
  • Excellent organization and communication skills

POSITION/TITLE:  Rehabilitation Consultant

AGENCY PROGRAM:  Rehabilitation Services

DESCRIPTION:        
Assist people with mental illness on their journey to recovery by:

  • Counseling clients in assessing their needs and developing plans of service
  • Providing outreach, case management, community support and psychosocial rehabilitation services as needed
  • Guiding preparation and skill-training for employment and or independent living
  • Coaching on and off the job site
  • Coordinating with business/training sites, employers, other providers, and other team members to provide on going support
  • Keep Rehabilitation Director informed at all times
  • Actively participate in agency meetings and trainings and in workshops and conferences as approved and arranged
  • Perform other duties as assigned

QUALIFICATIONS:
BA in Human Services or related field and experience in serving people with mental illness preferred.

  • Must possess excellent organization skills, sensitive, patient, responsive nature, excellent communication skills

POSITION/TITLE:  Vocational Coach

AGENCY PROGRAM:  Rehabilitation Services

DESCRIPTION:        

  • Assist, teach, and demonstrate vocational skills to clientele on and off Transitions sites.
  • Report progress and issues to rehab staff, employment team, and Rehab Director
  • Problem solve task completion difficulties with clientele, employers, and rehab staff.
  • Uphold all client rights as identified in agency policies.
  • Provide for the safety and well-being of all clientele, staff, visitors, and customers of the agency.
  • Keep Rehab Director informed at all times.
  • Actively participate in agency meetings and trainings and in workshops and conferences as approved and arranged.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • Must be 21 yrs old.
  • High school degree and two years experience providing services to persons with disabilities. Associates degree preferred.
  • Must possess excellent organization skills, sensitive, patient, responsive nature, excellent communication skills.
  • Current valid driver’s license and proof of insurance.

  • Basic computer knowledge required.

 

POSITION/TITLE:  Psychosocial Rehabilitation Coach

AGENCY PROGRAM:  Psychosocial Rehabilitation Services

DESCRIPTION:        

  • Assist, teach and demonstrate psychosocial skills to clientele in Transitions’ Take 2 program and other agency programs
  • Provide Medicaid and non-Medicaid services to clients, as needed
  • Collaborate with rehab consultants regarding client services, progress and assessment
  • Uphold all client rights as identified in agency policies
  • Actively participate in agency meetings/trainings and workshops or conferences as approved and arranges
  • Keep supervisor informed at all times
  • Read and responsibly follow/use all guidelines/information in Transitions Policy Manual, Procedure Manual, and Program Manuals
