               Service Structure                                   

Transitions Mental Health Services Staff works with each individual receiving services to identify personal goals and barriers and to create an individualized plan based on the individual’s own priorities and desires. Participation in Transitions Mental Health Services programs may be long or short term, depending on the needs and desires of each individual served.

 

                Counseling/Coaching                           

Services include both private and group opportunities for working through issues and obstacles that negatively impact an individual’s ability to live a productive and satisfying life.

 

                Youth Services                                        

Opportunities for youths, 15 years of age and older, challenged by mental health issues to develop and achieve their fullest potential through life-skills, education, counseling, and vocational training.

 

                  Employment  Services                          

Employment skills development, vocational counseling, job search assistance, application and resume completion, interview training, general and specific job skills training, counseling, on and off worksite support, and assistance in coping with, and working through, job-related challenges.

 

                 Residential Services                              

Sixteen-bed residential facility provides transitional housing for persons needing assistance with developing/improving the skills they need to successfully live independently in the community.

 

             Consumer Engagement                             

Unique consumer-driven approach to recovery from mental health issues through support, advocacy, and education, and an individualized, integrated recovery process.
