              Service Structure                                       

Service Structure – Transitions strives to work with each individual served to identify personal goals and barriers and create an individualized plan based on one’s own priorities and desires. Participation in Transitions programs may be long or short term, depending on the needs and desires of each individual served.

 

          Vocational Rehabilitation                               

Vocational Rehabilitation – Provides skills development and counseling for persons wishing to attain and maintain employment. Individuals learn coping and employment skills in agency-owned businesses/training sites, and in partnering community businesses.

 

                   Counseling                                             

Counseling/Coaching – Provides both private and group opportunities for working through issues and obstacles that negatively impact an individual’s ability to live a productive and satisfying life.

 

                Youth Services                                          

Youth Services – Provides opportunities for youths, 15 years of age and older, challenged by mental health issues to develop and achieve their fullest potential through life-skills, education, counseling, and vocational training.

 

             Consumer Engagement                               

Consumer Engagement - Offers a unique consumer-driven approach to recovery from mental health issues through support, advocacy, and education, and an individualized, integrated recovery process.

 

                   Residential                                             

Residential – Provides respite, short-term, and longer-term care living facilities for persons needing assistance with developing and improving the living and coping skills they will need to successfully live independently in the community.

 

                  Employment                                            

Supported Employment – Provides assistance in attaining and maintaining community employment. Assists in job search, application and resume completion, interview training, job search assistance, general and specific job skills training, counseling, on and off worksite support, and assistance in coping with, and working through, job-related challenges.
